Abstract :
The clinical judgment to retain periodontally compromised teeth or proceed with extraction and implant-based rehabilitation extends beyond comparative efficacy data and incorporates multi-level determinants: patient-, dentition-, and site-specific risk factors; periodontal, restorative, endodontic, and occlusal considerations; the patient’s preferences, finance and life events; and the clinician’s expertise and training. We will review modern periodontal treatment approaches and maintenance strategies that can shift prognosis, including their effectiveness and cost-efficiency. In addition, we will explore the biological consequences of periodontal tooth loss, implications in implant rehabilitation, challenges and needs for implant site development, and strategies to minimize complications.
Learning objectives :
- Appraise contemporary periodontal therapies and maintenance strategies that modify tooth prognosis, integrating multi-level risk stratification and restorative considerations to guide retain-versus-replace decisions.
- Explain the biological sequelae of periodontal tooth loss, anticipate alveolar dimensional changes and implications in rehabilitation.
- Plan evidence-based implant rehabilitation pathways, including site development, risk based maintenance, and strategies to minimize complications.
